Infliximab Rapid Infusion Protocol in Pediatric Patients

Does anyone have experience in using accelerated infliximab infusion in pediatric patients and are willing to share their protocol? I see a lot of data for adults, but all specify that it is for patients >18 y.o.
Much appreciate any info you can share.

Missing Medication data/benchmark

Does anyone use a benchmark to compare data as it relates to missing medications as reported by nursing?

We continue to have discussions related to MM.05.01.11 EP 3 - hospital dispenses meds within time frames it defines to meet patient needs.

We have collected data on re-dispenses of medications to assist in quantifying the concern. We have improved processes to help reduce the number but would like to be able to compare this to a benchmark if possible. Is anyone familiar with a benchmark or can you share how you review data?

Patient specific medications and BCMA

Can you describe your approach when a patient specific medication (common practice in peds) is "accidentally" discontinued and then immediately re-ordered (same dose, same frequency) with respect to bar code scanning?

When the above happens the barcode on the dose already in the ADC will no longer scan ---we are seeing that the RN won't know this and then calls pharmacy saying the dose doesn't scan.

Curious as to other people's process.

"Kits" with controlled substances

Asking for pediatric experience/children's hospitals only please:

Do you store intubation kits in your ADC's that have controlled substances in it?

What is in your kit?

Which units are they stored in (meaning do they exist on general medical floors or only in ICU's/ED)?

What is the return/waste process after use of these kits?

Concerns with Chemotherapy tubing and bags

We have had 2 instances of chemotherapy tubing and bags becoming pressurized to the point that the tubing and bag flatten – preventing the infusion of the entire dose. Our Alaris pumps beep “air in line” and won’t allow the complete dose to be infused. In both cases the chemotherapy bags were rigid plastic, as opposed to a softer bag like a 1,000 mL Normal Saline bag. Nursing staff have recommend pharmacy inject more air into the rigid plastic bags when prepping the chemotherapy, however it is unclear if this would impact the pressurization that is causing tubing to collapse

Go-lytely continuous infusions

Hello.
I've identified a unique workflow and am interested to see if other organizations have encountered this. We have some providers that are ordering continuous infusions of go-lytely (max rate of 1,200 mL/hr in some instances) to be run via an enteral tube. If your organization sees these types of orders, what does administration look like? I think our actual order on the MAR is fine but the workflow for nursing to achieve this rate at the bedside is excessive in its current state.

ADC Auto Log Off Interval

We recently implemented Pyxis in our Emerg department. They're asking to extend the auto-log off interval for the critical care and trauma devices as the're finding they need to keep running back and forth with medications, (during code situations) and they need to log in each time. The crash trays aren't kept in Pyxis, but they're accessing Pyxis for things like ketamine, fentanyl, midazolam, etc. Currently, it's set to 60 seconds for all devices. Have any sites extended this interval beyond 60 seconds? Do any sites have it set to less than 60 seconds?

LET gel in ambulatory clinics

We are currently looking for a new way to procure LET gel for our urgent/immediate care clinic network. I'm curious how those of you with ambulatory clinics are supplying this compounded medication.

(1) 503b compounding pharmacy? If so, which one? Some report they are unable to distribute direct to clinic (can only dispense to a licensed pharmacy).
