Medication Reconcilliation Ambulatory Services

In planning to implement medication reconciliation at Covenant Health facilities in the ambulatory care setting, we are trying gauge what best practice would be for inclusion/exclusion criteria for the full process of medrec in ambulatory clinics and for perioperative clinics. What decision algorithms were used and was supporting literature for the decisions incorporated?

Tranexamic acid on acute care floors

Hello,

My institution uses TXA for trauma-transfusion, spine/large ortho surgeries, and OB emergencies. Thus far we have limited infusions to our critical care areas within our IV administration guidelines. I am exploring liberalization toward acute care units in some manner. Polling to see if others allow TXA use on non-ICU floors.

Phenol Use

Good afternoon all,

I am reviewing our hospital's use of phenol in the OR, and was wondering if this practice continues at other institutions. If your institution previously used phenol, but is no longer performing this practice, would you be willing to share your journey away from the use of phenol? If your institution currently uses phenol, would you please consider answering the brief survey questions below? RePackaging of Morphine and OxyCODONE oral concentrate liquid - is there a BUD beyond USP 795

We need literature to support repackaging morphine and oxycodone oral concentrate liquid from multidose bottle to standardized oral syringes doses (e.g. 5 mg/0.25 mL). Manufacturer has no repackaging data and USP 795 states 14 days REFRIG BUD for aqueous compounded nonsterile oral preparations. Need information to support extended BUD in oral BD syringes. Thank you.

Override review policy

As per TJC, as of January 2018, pharmacy needs to have a policy addressing pharmacy review of medication overrides. How often are other hospitals are reviewing medications overrides? Is it monthly, quarterly, etc? Are any hospitals that have an override policy willing to share?

SWFI and NS Diluent Shortages

We are having difficulty obtaining both SWFI and NS vials to use for medication reconstitution.
I am having difficulty coming up with creative solutions and effectively discouraging the use of NS flush syringes for med reconstitution (particularly for some of our hospitals that do not have 797-compliant IV rooms). Would love to hear ideas from others!
