We are implementing Cerner and looking at creating integrated dose checking and dose alerts.
Interested in know if you you utilize this and if so what type of variance you have placed for high dose alerts. We are trying to balance safety with alert fatigue

Wondering if others are experiencing any product issues / complaints with the Avet manufacturer Propofol vials. Have had a couple reports of vial stoppers popping into the vial with puncture and product spraying out with puncture.

Hello Everyone,
Years ago we use compound alteplase 1 mg syringes and freeze for 6 months pre USP 797. I realize the package insert states 8 hour stability. There are studies that support compounding stability and cyro storage of -30C for 22 weeks. Any infor would be appreciated.

We are seeing more and more LR being used for maintenance fluids and issues with the nurses hanging piggybacks that are incompatible. Does anyone have a good list of meds that cannot be Y-sited with Lactated Ringers?

Do any institutions unit dose/blister pack medications that the manufacturer explicitly states to keep medication in original packaging? If so, what supporting information and BUD is attached to the newly packaged item?

Hello Med Safety colleagues.

Our institution currently uses norepinephrine 4mg/250ml (16mcg/ml) and 16mg/250mL as our standard and maximum concentrations, respectively.
Unfortunately, due to the number "16" associated with both, we have had multiple pump errors reported.
We do not have pump integration at this time, and we dose norepi in "mcg/min."

Hello colleagues,

We are consistently seeing errors with extended infusion antibiotics NOT being ordered for the appropriate administration duration. We are an Epic institution and have two orderable options (i.e. Zosyn IV and Zosyn IV Extended Infusion Panel).

Does anyone have any effective solutions to assure accurate ordering? Thinking additional language in the orderable display name to target use or order instructions?

Does anyone have a policy and/or a procedure on how to handle patients on IV bisphosphonates? How long do you have patients wait between dental procedures and infusions and what steps have you implemented to prevent patients from receiving these medications too soon after a dental procedure?

Has anyone transitioned to sending oral solutions in Enfit oral syringes only. Including repackaging unit dose oral cups supplied by manufactures to assist with oral administration at the bedside?
