additives to PD fluid

What process do you use for addition of antibiotics and/or heparin to peritoneal dialysis fluids?
What BUD do you assign to PD fluids with additives?
Do you require the PD bag to be warmed prior to addition of medications?

We require nursing to bring a warmed PD bag to pharmacy for the addition of medication, and consider the prepared bag to be for immediate use.

Despite the PD fluid being stocked in the ADC, our EHR won't allow removal of the PD fluid via the order due to the order having additives. We do not have PD fluids on override in the ADC to reduce the risk of the incorrect fluid being pulled.
Therefore our nurses need to come to the pharmacy, pick up the bag, return to the unit to warm the bag, bring it back to pharmacy for additives, then return to the unit to administer.
We also have a challenge when receiving a request from nursing to prepare the bag without additives for pick-up from pharmacy, as we cannot label it with an EHR-generated label, as the bag does not have the additives yet.

We are looking to streamline our process. Any input appreciated.
