Adverse Drug Reaction Surveillance Programs

Hello everyone,

My name is Nick Fritz, and I am the current PGY-2 Drug Information Pharmacy Resident at the Medical University of South Carolina.  As part of a project to improve our processes for monitoring adverse drug reactions (ADRs), I have developed a survey to assess the methods other institutions use to streamline identification and documentation of ADRs.  If you have a moment and can fill out the survey found at the link below, I would greatly appreciate your help.  The survey should take between 5 and 10 minutes to complete.  If there is a better person in your organization to complete the survey, please feel free to forward the link along.
