"Beyond Blame" Video

Our hospital shows the “Beyond Blame” video to all clinical staff during orientation. The video was made in 1997, and I came away with the message that CPOE and bedside barcoding will prevent these catastrophic errors. If used correctly, CPOE and bedside barcoding will prevent many of these errors, but I continue to find clinical staff workarounds and overreliance on technology has diminished the impact of these tools, unfortunately.

Is there a more recent video that we should be using?
Don’t get me wrong - I really like 95% of the “Beyond Blame” content, but I don’t want the video to give our staff a false sense of security from our implementation of these technologies. I’d love to see an updated version of the video that includes examples of incidents from facilities that had these technologies in place , but an error still occurred because of staff workarounds, etc.
