comparing med error rates

Hello!

I recently saw a headline in one of my pharmacy journals, magazines or newsletters reminding us the concerns and barriers with comparing medication error rates between facilities. I recall seeing the headline and ignoring the article because it is not something we do at our facility as we understand the differences in safety cultures, the lack of standardization, different processes, etc between facilities. Yesterday, however, I was challenged by one of our providers who is concerned with our facility's error rate given the size of our critical access hospital. Although the majority of our reported events are near misses, he is missing the bigger picture of more reporting representing a safer culture.

I am having a hard time finding the headline that crossed my desk- does anyone recall a recent publication or headline on this topic? (I am not having any luck with Dr. Google) Can anyone tell me the most recent ISMP article on the topic of comparing error rates/benchmarking error rates with other facilities? I would love to share this with information with my provider.
