concentrated potassium vials

Good afternoon,
How does your institution/system handle potassium chloride injection for:
1- peritoneal dialysate additive for inpatients- does pharmacy prepare, or do you allow PD nurses to prepare?
2- maternal fetal medicine - for selective reduction- do you allow vials or ha?
3- cardiac perfusion- do you allow vials (with safegards??) or do you have an alternative way of dispensing
Appreciate sharing of strategies to remove potassium chloride vials from the areas.
