We are evaluating our policy on double check of insulin products. We had converted to pharmacy drawing up Lantus and other long acting insulin products in our IV room using camera compounding technology but a double check still remains for nursing on all insulin. This double check (long-acting insulin products excluding U500 as separate policy) is mainly to ensure the syringe is appropriate for the patient and it is drawn up to the correct dose however they have no vial to compare to. Has anyone removed or not have a double check on insulin drawn up by pharmacy? If no double check, any other practices in place to prevent any insulin errors and keep a high alert status. Feel free to respond if you have a practice such as ours and have not removed the double check for long acting insulin products. We do not plan to change the double check for our short acting insulin products at this time.
