Duplicate therapy- to alert or not to alert?

I am hoping to find out if anyone has had success in turning on duplicate therapy alerts without causing too much alert fatigue?

For context, (I am on Epic) we currently have the out of the box duplicate alerts shut off, but have built a handful of high risk duplicate rules that will alert for certain drugs/categories. This doesn't seem to be enough to catch all potentially harmful duplicates.
