Has anyone dealt with this topic "ISMP Special Alert! - Short period of Surescripts medication history inaccuracies"

If an institution has dealt with the Surescripts medication history inaccuracies or if no actual patients, but reviewed process for if had to respond. Reviewing what we would for a potentially affected patient:
1. Review Surescripts medication claims listed
2. Verify if patients pharmacy(s) filled the listed Surescrips claims and/or medications in history list
3. Review EMR for any mention of outliers not validated by the aformentioned pathway

Using this approach:
1. Is there any other modalities other institutions are utilizing, other than asking the patient?
2. Do you have health professionals identified for input regarding Surescripts data or questions therein?
3. Whom would you task for this review, Pharmacy, Providers, IT Department, all or some?
4. Any additional comments?
