Locations in Patient Safety Reporting System

We currently categorize the location of our events by where the patient is located, rather than the location of where the error occurred. We are considering the addition of pharmacy locations within our reporting system to allow better event capture and trending of errors that occur within the pharmacy.

1) Do you have pharmacy-specific locations within your patient safety reporting system?
2) For larger institutions, How specific are your locations? (inpatient pharmacy, satellite pharmacy, outpatient pharmacy; or more specific, such as "OR Satellite pharmacy", "SICU satellite pharmacy", "IV room", "Central Pharmacy", etc., etc.).
