Hi there!
We had an recent concern escalated from our Children's Hospital around the dispensing of LORazepam from a retail pharmacy setting when it comes to sedation weans. (Children that had been on long term sedation in the ICU setting may require a somewhat prolonged wean from their medications, even at discharge.)
It was noted that outpatient/retail pharmacies are NOT dispensing / will not dispense the specific amount needed to complete the wean, but are rather required (by law?) to dispense the entire 30 mL bottle of concentrated lorazepam due to the bottle stating "Dispense only in this bottle and only with the calibrated dropper provided"?
* Have you heard of this safety concern before (risks around overdose / over-dispensing / diversion or abuse)?
* Are outpatient pharmacies are required to comply with this?
* Any idea on why manufacturer's would be putting this on their products?
* Any solutions or recommendations that your organization has developed?
Thanks so much in advance!
Emily
