Lorazepam shortage and alcohol withdrawal protocols

Hello,

I wanted to reach out to see if anyone has increased the use of Midazolam while dealing with the severe lorazepam shortage. We are considering this and being very conservative with dosing. We are also increasing monitoring.

We are simultaneously working on a long term goal of minimal to no BZD protocols and the use of other options like gabapentin and phenobarb that we've seen in the literature. But those are gonna take longer (EMR orderset build, P&T approval, etc). This is also to assist in a permanent move away from lorazepam considering how much we waste with out of refrigerator dating.

Would you be willing to share what is happening at your institutions?
