Medication Administration Teaching

Hello All,

We are seeing an increase in reporting of general medication administration errors (not charting medications administered, administering meds but not charting, forgetting to unclamp secondary lines, incorrect routes of admin). We are attempting to determine if these are incorrect habits formed from COVID surge or something bigger. We are also currently into our HRO journey ~1 year, so may just be increased reporting of these errors and not necessarily a change.

Questions:
1) Anyone else seeing these trends post COVID surging?

2) For general education around medication administration standards, does anyone have suggestions for what type of education they perform with nursing and at what frequency? Our staff receive one formal piece of education when they are hired, but nothing formally after that.
