Medication Context for obese pediatric patients

Good morning all,

At our hospital we had near misses where obese pediatric patients ordered weight based meds with an unsafe dosing due to patient weight.

Currently we have following context in EPIC
- Any patient < 15 years: pediatric context
- Patients 15-21 years AND <= 40 kg: pediatric context
- Patients 15-21 years AND > 40 kg: adult context

but these doesn't catch patients <15 years old and obese (we recently had 14 yo and +120kg). Can you please share how do you account for these patients to make sure their medication dosing is safe and appropriate?
