Medication Event Rate

I have had a request from our operational pharmacy leaders to report medication event rate as a KPI. While our entire health care system reports serious safety event rates for all events, we have not previously reported a medication event rate. I'm struggling with creating a meaningful numerator and denominator....I'm using total events reported for numerator and have used various denominators such as APD and total dispenses calculating the medication event rate per 10,000 APD or 10,000 dispenses, either as monthly rate or as a 12 month rolling rate. I'm curious what other health systems are doing for a similar KPI? Any insight or shared experiences would be very much appreciated!
