Medication History - ED & Radiology Patients

We are an HFAP accredited institution. For the medication reconciliation standard (25.02.03)how does your facility interpret "EMERGENCY DEPARTMENT: A complete list of current medications is to be obtained for Emergency Department patients." Are getting a complete list (name, dose, frequency, etc...) for all ED patients, even those who are for sure NOT being admitted? We are embarking on a process improvement project at my organization re: medication histories and looking to see how others define this. This also applies to radiology outpatients presenting for CTs, etc... We currently do not have medication historians. Thanks!!
