Override review policy

As per TJC, as of January 2018, pharmacy needs to have a policy addressing pharmacy review of medication overrides. How often are other hospitals are reviewing medications overrides? Is it monthly, quarterly, etc? Are any hospitals that have an override policy willing to share?
