Patient specific medications and BCMA

Can you describe your approach when a patient specific medication (common practice in peds) is "accidentally" discontinued and then immediately re-ordered (same dose, same frequency) with respect to bar code scanning?

When the above happens the barcode on the dose already in the ADC will no longer scan ---we are seeing that the RN won't know this and then calls pharmacy saying the dose doesn't scan.

Curious as to other people's process.
