We are having issues with physicians not consistently addressing medication order renewal before the order falls off the MAR. This should be a physician responsibility, but we have been asked to determine the feasibility of pharmacists utilizing an EHR (EPIC) workbench report that shows all the med orders expiring in the next 3 days and then contacting the physicians. I ran the report to see what our pharmacists would be dealing with and there are over 100 different providers and hundreds of patients that show in the report. I've asked to see if the report can be run and then routed to physician group/team inbasket folder or something similar but I do not know if that is feasible at this time.
1. Do any of you require/expect your pharmacists to contact providers when medication orders are expiring?
• What challenges have you experienced?
• How are physicians held accountable if a pharmacist contacts them but the order does not get addressed before it expires?
2. For those who do not have pharmacists contacting providers about expiring meds, do you have anything in your EHR system that the physicians find useful to remind them to address the expiring med?
