It seems that most medication safety pharmacists/staff report to the Pharmacy department. I wanted to know what is the reporting structure of the medication safety program? (e.g. is there a reporting structure for the program to a department in the organization vs the Pharmacy & Therapeutics Committee? Thanks and happy holidays.
