RN dual sign off for subQ insulin

We have had several requests to re-evaluate our dual sign off medication list for nurses and are currently focusing on subcutaneous insulin use. We are trying to gather information about what others are doing, if anyone is willing to contribute by answering the following questions:

-Do you require dual sign-off for subcutaneous insulin administrations?
-If you do not require it, have you implemented something else in its place to ensure the proper dose is being administered?
-Also, do you have a quality control test in place in order to be able to detect errors?
-Is insulin stocked in your automated dispensing cabinets?
-If so, do you require any additional sign-offs or anything else at the machine?

Please also include hospital information (location/name).
