Standardization of Leur Slip Syringes for SQ/IM Doses – Feedback on Pediatric Dose Adjustments

Dear Med Safety Board,

As part of our effort to standardize practices across our pharmacy departments, we are implementing the use of leur slip syringes for dispensing all SQ/IM doses, including those for pediatric patients. Under this new process, pharmacy will dispense medications without needles attached, and nursing will add the appropriate needles at the bedside.

One area of concern we’ve identified is the potential for medication loss within the needle for very small doses, particularly in our pediatric population. To ensure dosing accuracy, we’d like to understand the current practices across departments:

• Do you currently add a small extra volume to account for potential medication loss within the needle?
• If so, how much additional volume do you add to these doses?

Your insights and any established protocols will be invaluable as we work to standardize and optimize this process. Thank you for your collaboration.
