Stocking Errors

We continue to see wrong-bin stocking errors in our medication carousel, and some of these errors are making it all the way to the Pyxis machines.

Our carousel bins do not have lids, so medications can easily be placed in an adjacent bin. Barcode scanning is required for stocking and returns, and standard work is to handle one medication at a time. Pharmacists also use barcode scanning when checking dispenses and Pyxis replenishment pulls, but each individual tablet, vial, or package is not scanned.

We have shared events, provided coaching, and reinforced standard work, but the errors are still occurring.

Has anyone made workflow, technology, or bin-design changes that helped reduce these types of errors? I would appreciate hearing what has worked at other organizations.
