Titratable Drips_Reconciling after transfer from OR

Everyone's favorite hot topic, with a slightly different twist...
Do you have a policy or practice to share reflecting expectations for reconciling orders for titratable medication infusion rates after a patient transfers/returns to a unit? For instance, a patient returns to the ICU from a procedure in the OR with 1 or more titratable drips running. It takes some time to get some patients stable and settled on the unit. When would you expect providers to update the patient's orders so that the ordered rate matches the administration rate? If you have a policy or practice you would be willing to share, that would be great.
