Two February Dates You Don't Want to Miss

High Alert Self Assessment Deadline: February 28, 2018

Healthcare facilities that would like to participate in the first ISMP Medication Safety Self Assessment for High-Alert Medications must submit their data by February 28, 2018. This is a unique opportunity to evaluate your best practices related to high-alert medications and obtain weighted scores to compare with demographically similar organizations. Participation can also help meet requirements for managing high-alert medications from regulatory and accrediting agencies. Have you eliminated injectable promethazine?

ISMP is looking for organizations that have fully implemented the new ISMP Targeted Medication Safety Best Practices for Hospitals #13 (below).  If you are able to share your experience(s), please contact Allen Vaida at avaida@ismp.org

#13- Eliminate injectable promethazine from the hospital.

  • Remove injectable promethazine from all areas of the hospital including the pharmacy.
  • Classify injectable promethazine as a non-stocked, non-formulary medication.
  • Implement a medical staff-approved automatic therapeutic substitution policy to convert all injectable promethazine orders to another antiemetic.
  • Remove injectable promethazine from all computerized medication order screens, and from all order sets and protocols.

ISMP Releases 2018-2019 Targeted Medication Safety Best Practices

This week ISMP has launched an updated version of its Targeted Medication Safety Best Practices, which include two revised and three new  practices for 2018-19.

The purpose of the Targeted Medication Safety Best Practices for Hospitals is to identify, inspire, and mobilize widespread, national adoption of consensus-based best practices for specific medication safety issues that continue to cause fatal and harmful errors in patients, despite repeated warnings in ISMP publications.

New ISMP Educational Opportunities

ISMP wishes to announce two new educational opportunities for MSOS members:

  • Third Free ISMP Video Newsletter
    • ISMP has launched the third in a series of “video newsletters” being produced in partnership with the Temple University School of Pharmacy. The videos feature short interviews with nationally recognized experts on current medication safety issues, and provide a summary of top content from the biweekly ISMP Medication Safety Alert! newsletter.   •  new ISMP/OE Medication Safety Focused Just Culture Certification Course.
    • Effective error prevention depends on having a Just Culture. Outcome Engenuity (OE), in partnership with ISMP, will offer a Medication Safety Focused Just Culture Certification Course in 2018 on February 21-22 in Phoenix, AZ. The course will train healthcare providers to lead their organizations through a culture transformation, and also includes a yearlong subscription to the Just Culture Assessment Tool (JCAT). ISMP Self Assessment Deadline Extended to February 28, 2018

ISMP is extending the deadline date for healthcare facilities to submit their findings for its new self assessment on high-alert medications from December 15, 2017 to February 28, 2018. Each section of the ISMP Medication Safety Self Assessment for High-Alert Medications can be completed and submitted to ISMP separately when ready. ISMP is extending the deadline to ensure that organizations have enough time to participate in the full assessment by evaluating all the categories of high-alert medications used in their facilities.
