ICU Medication Reconciliation

Hello everyone! I was hoping to get some feedback regarding your ICU med reconciliation processes.

Currently our ER pharmacists do med rec from 0600-0030, so we have about a 5.5 hour gap in coverage. And of course, sometimes they are pulled away for other clinical activities so not everyone who comes through the ER has their med rec done by a pharmacist.

We have an ICU physician who wants a way to capture the ICU patients whose med rec was missed. We do not want to pull away our ICU pharmacist for this since they need to focus on their other ICU clinical duties; nor do we have the resources to say pharmacy will help with med rec for all ICU patients who were missed in the ED.

Just wondering if anyone has a process for capturing these patients and who is involved with completing the med recs? Any visual reminders or alerts in the chart? Does anyone have success with ensuring that nurses do this when transfer reconciliations are done? Or any other ideas?
