B&O Suppositories in Pediatrics

Hello!

We have a request to add Belladonna Alkaloids-Opium (B&O) Suppositories to our formulary. Over 20 years ago we had a patient safety event regarding the use of this suppository in a pediatric patient. Has anyone else experienced serious safety events at their system with the use of B&O suppositories in pediatric patients? For those that have these on formulary, do you have specific protocols/restrictions for use? I've been told ISMP sent out a safety alert involving the use of B&O suppositories in pediatric patients a while ago, however I am unable to find that alert.
