NICU 3-mL Doses - Syringe Size and Pump Flow Rate Limitation

Does anyone have any best practices around ensuring 3-mL syringes are used to prepare 3-mL NICU doses?

We’ve had incidents of 3-mL doses dispensed in 5-mL syringes. The ordered rate was less than 0.1 mL/hr. Because the Alaris syringe pump cannot deliver a flow rate lower than 0.1 mL/hr from a 5-mL syringe, the doses had to be sent back to be prepared a second time in 3-mL syringes.

We are thinking about including syringes as components to be scanned when Dispense Prepping NICU mixtures in Epic but are running into some workflow challenges with that.

Separate pumps for IV infusion and enteral feeds in NICU?

We are evaluating why our NICU isn't able to reach our goal DERS compliance and found out that some staff members haven't been switching profiles in the pumps from IV infusion to Enteral feeds and are using the basic mode in the IV infusion for feeds. Although low, there is a risk of enteral feed been given IV if the profile isn't changed. We are assessing the need to have a totally different pump for our enteral feeds. Wanted to know what other NICU's in the community do.

ENFit Oral Syringes

Our NICU wants to switch to NeoMed ENFit supplies.

After reviewing ISMP articles and other documentation, I'm concerned that this may not be the safest path to go. Even with the low dose tip, it seems there are concerns dosing variances higher than legacy male tip oral dose syringes. Yet, when I go to the manufacturer's website, their main focus seems to be neonates.

Any thoughts from any of you who have already been down this road?
