We are updating our High Alert med policy and I'm wondering how other facilities differentiate between high alert meds that require an independent double check and the ones that do not. ISMP's list is extensive and states that the double check is not always optimal. But, I feel like the staff hears "high risk" and assumes double check for the most part... Also, wondering if people are still including warfarin?

I would be interested in what your policy is if you are using DOACs for pediatric patients. Thanks in advance. Rivaroxaban is our current formulary item. We may add it to our High Alert Medication list soon...

Question for the group regarding High Alert Medications.

Does your organization currently have any sort of "identifier" or visual cue, etc. built into the EHR to alert end users to the fact that a medication is considered 'high-alert'? if so, would you mind providing information on how you have this set up / what it looks like.
