Calculating ideal body weight

Does your order entry system automatically calculate IBW for patients who are under 5 ft tall?
If so, what formula does your system use?
Do you have one formula for patients 5ft tall or greater and a different one for patients less than 5 ft tall?
Do any of you use the CDC Growth charts to calculate IBW?
