Expensive nonformulary home medications

Can you please share how your facility deals with expensive nonformulary home medications?

- Do you allow pts to use their own home medications or do you try to obtain all medications that are deemed medically necessary?
- What happens if this is an item that is drop ship or only available through specific wholesalers/manufacturers?
- What happens if the pt does not have the medication at the hospital? What If the family is unable to bring it in? Do use a courier of some kind to go to the pt’s house to get the medication?
- Is there any consideration of cost in your decisions to purchase vs obtain pt own?
- Is there any consideration of the formulations or storage of items (refrigerated items or injectables)?
- How do you ensure that these high cost medications are returned to the patient at discharge?
