Fentanyl infusion concentrations in NICU and Peds

ASHP Standardize for Safety and the Vermont Oxford Network recommend fentanyl 10 mcg/mL and 50 mcg/mL. Have any other institutions changed to this? We are struggling to see how to make that work without causing fluid overload. Especially in our NICU patients who require parenteral nutrition.

Have any other institutions made the switch to just having fentanyl 10 mcg/mL and 50 mcg/mL infusions?
