Fentanyl infusion vials' stoppers pushed into vials

Fentanyl infusion vials' stoppers pushed into vials

We use Hikma fentanyl 2500 mcg/50 mL vials for fentanyl infusions and the nurses insert tubing directly into the vial and hang the vial. Recently there has been an increase in reports with stoppers being pushed through into the vials with the same Hikma vials. May be a specific lot, C26110, that is causing the issue. Not sure. Anyone having this same problem?
