We are all aware of ISMP's targeted safety best practice 6 to avoid glacial acetic acid, which has been archived since 2020. Attached is the NAN alert from 2013.
I always thought we had this best practice squared away, since we don't use glacial acetic acid. However, I realized that our OR uses acetic acid 4%, which we obtain for them from a chemical supply company, since our wholesaler only has the 0.25% product.
This chemical supply company has many listings for glacial acetic acid products, which the user sees by searching for "acetic acid". Although we would never intentionally order glacial acetic acid, we could easily end up ordering it by mistake, especially if someone is covering for our regular buyer.
For those who use acetic acid 4%, are there any medical supply companies that offer it that don't pose this risk?
Is anyone using household vinegar (5%) from a grocery store?
What other strategies do people use to mitigate the risk of accidentally ordering glacial acetic acid?
