Hemodialysis Catheter Lock

Good afternoon,
Our pediatric outpatient dialysis centers see both children and young adults and have changed practices several times over the years in regards to lock solutions for their hemodialysis catheters. Currently they are using alteplase to each lumen at each session. I wanted to poll the group on the following:
1. What population(s) is/are seen in your outpatient dialysis center(s)?
2. What is the standard lock solution used for hemodialysis catheters?
3. If heparin, what concentration are you utilizing?
4. If alteplase, do you have a maximum dose? If so, how do you account for the remainder of the catheter volume?
