Home insulin pumps

We are trying to develop a policy and work instruction for how to handle patient's own insulin pumps in a perioperative setting and also for patients that are admitted acutely ill and on an insulin pump at home. I would appreciate any information and/or resources that anyone could provide. Do you have standard instructions for when to adjust or turn pump off prior to surgery/procedures? How and when to transition to intermittent sq insulin for inpatient use? Do you allow the use of patient's home continuous glucose monitoring systems? etc. Thanks in advance!
