insulin pen

Just trying to figure other hospitals practices with insulin pen vs. vials.

•Do you have any insulin pen that you use at your institution
•If yes, what pens do you use?
•If Yes, where do you store them? In Pharmacy or ADC?
•How do you label it?
•What Bar Code is used to administer mediations? Patient specific bar code or
NDC from the Pen.
*If you use vials, where do you store? Pharmacy or ADC?
*If you use vials, how do you dispense, draw up patient specific doses or nurse draw up doses on the floor?

Thanks for your help
