Intra-facility transfer of POM and/or Patient specific medications

I am reaching out to inquire if any organizations currently using Epic have a system solution to prompt the appropriate transfer of medications when patients change level of care within the hospital?
Currently, within our organization the onus is on the responsibility of the out-going RN to ensure medications, whether patient own supply used during their stay or medications sent from pharmacy and stocked within the Omnicell patient specific bin (PSB) being transferred to the receiving unit.

Do you have any prompts within the EHR to nursing to help drive a reminder to ensure medications are transferred (any required documentation, navigators used, tasks fired, etc)
