Patient Specific Lantus -- Two Weeks Dating

Recently, our facility has located limited data supporting the stability of Insulin Latus, for two weeks at a temperature of 32◦ to 40◦ F. Daily Lantus doses are prepared, patient specific, in the pharmacy, and delivered to nursing units.

Are any institutions recycling insulin doses, either by modifying the dose or relabeling the syringe for use in a different patient, for a period of up to two weeks? I wanted to inquire if any other facilities have adopted this practice, a similar process, or could provide any additional insight or polices regarding the use of Lantus for two weeks. Thank you.
