Pediatric intracatheter alteplase

Hi
How are other institutions ordering, dispensing and administering alteplase for catheter occlusions in pediatric patients? We currently have a varied practice (some units have alteplase 2mg vials in their ADC, some units have our IV room draw up doses) and our EHR orders don't really match up to how it is being administered. Our policy states that patients LESS THAN 30 kg administer 110% of the catheter volume, but our MAR has a flat dose of 0.5, 1 or 2 mg. Some of our pediatric doses can be as small as 0.08 mL (0.08 mg) and our ordering providers do NOT know the catheter size/volume when placing the order.

We have epic as our EHR and would love to hear how others are doing this!
