Pharmacy Driven Discharge Med Rec

We plan to focus our energy with regard to safety in the TOC-space, by increasing Pharmacy involvement in Discharge Med Rec by partnering with providers across our system. Who else is working in this area? We would like to eventually get to a VA-type model, but would like to see what is commonly in place.

Questions:
1) Is your discharge medication rec prospective or retrospective (before or after the provider)?
2) Which EMR do you employ? We utilize Cerner, so would love to speak to another Cerner facility.
3) What are expectations for documentation for this process?
4) Is this an expectation of every decentralized pharmacist or do you have a specialized team?
5) What are the quality metrics that are employed to track the efficiency and ROI of this program?
