Preferred Names on Labels

A discussion surrounding use of preferred names on labels (med labels, lab labels, name bands, etc) has resurfaced in our Med Safety group. During our discussions we identified several risks with including these, but also understand the importance of including particularly in the case of transgender patients.

- Have other institutions navigated the process of including preferred names on medication labels (and other labels)?
- Do you include for all patients?
- Are phonetic names placed as preferred names also included?
- Are patients allowed to update their preferred name, or are there restrictions to who can update?

We are an Epic institution.

Any advice or experience you could share would be much appreciated!
