REMS program

Hi,

I need insights/guidance on how to structure a REMS program to meet both pharmacy and provider regulatory requirements at a large hospital.

Few questions I have:

1. Do you have a REMS committee at your institution? If so, how many stakeholders do you have from the provider side? Do you have a point person for each REMS?
2. How are you keeping track of and documenting provider enrollment, provider education and patient counseling?

3.How did you incorporate EHR? Did you build BPA or LQL?

4.Which tools are you using to track the regulatory requirements?
