What precautions and reminders are present at your organization to ensure that medications are appropriately adjusted by pharmacy for renal insufficiency?
Some examples of precautions:
-Renal adjustment protocol/policy attached to medication orders that can require renal adjustment
-Medication warnings that automatically fire if renal adjustment is not completed
-Order panels/sets that direct the provider to the correct dose based on CrCl
-BPAs/OPAs/LMAs (not sure of the best criteria for these)
-Daily pharmacist review/iVent of medications requiring renal adjustment to ensure they are still applied correctly
-Note to Pharmacy during verification
