Safety event feedback loop

Hi everyone,

Does anyone have a really great process for providing feedback to the person who reported the event into your safety system? We recently started a bi-weekly patient safety meeting with a couple key stakeholders and I am suggesting that we implement some sort of process to follow up with the person who reported the event. What I thought of was a general Word Document that thanks the reporter, states what day this was discussed at our bi-weekly meeting, a comment (generic or not) about addressing the system around the error, and a general comment about event reporting and our culture of safety. The document could be signed by me if med event, others for non med events and ideally also include the unit manager. I would suggest that when the document is given to the manager for signature, then he/she could be responsible for giving to employee/reporter?

We are a small rural hospital and you would think that we had this process for a feedback loop nailed down. Small and rural also means that we all wear many many hats, and medication safety is no one specific's hat at this time.
