Surgical antibiotic timing

Hello Everyone,

Our hospital recently went live with weight based dosing for surgical antibiotics (vancomycin and gentamicin). We are running into dispensing delays because the patient does not get weighed until they enter the building. Then the pharmacy will verify the order and mix the dose causing a delay. Another delay falls into dispensing the dose to the right area. Our printed label will generate with a nursing unit location but the patient is no longer on the nursing unit and is in transport to the OR or cath lab. Does anyone have a protocol or workflow that they are willing to share to assist with this issue?
