Tenecteplase Storage in Pyxis

Colleagues,

My system (small-medium community hospitals) utilizes tenecteplase for both acute ischemic stroke and MI/PE indications. We currently have a "tenecteplase for stroke" physical kit in Pyxis. This has created confusion when tenecteplase is needed for PE (currently stored in pharmacy). In effort to support safe, timely, and indication specific dosing, particularly at one site when lacks 24-hour pharmacy coverage, I think a kit for PE/MI dosing is the safest storage strategy. The frustration being expressed is over the creation of multiple kits. I am concerned that stocking tenecteplase for this indication, despite adding clinical alerts, as a drug in Pyxis poses significant risk for a dosing error given alert fatigue and the absence of pharmacist coverage.
