Transfer patients ( from ER, OR , to Critical Care and Med Surg areas )

Good morning ,

Can anyone share examples of policies or processes for transferring patients requirements with active medications or patient specific medications?

Scenarios :
Patient transfer from ED to Inpatient units with active heparin drip, PCA, veletri infusion.

Patient transfer from Med Surg inpatient units to Critical Care units with patient specific medications.

Will you require always the nurse going with the patient?
